Background: Acute scrotum in pediatrics is an emergency characterized by the sudden or progressive onset of pain, swelling, and redness in the scrotal area. It is crucial to promptly diagnose and treat the underlying cause to prevent serious complications. Case presentation: A 19-day-old male, presented with 4 days of difficulty feeding, progressive irritability, a single episode of fever of 38.5°C, and on physical examination right scrotal swelling was found. The swelling was non-reducible, confined to the scrotum, and the testicle could not be palpated. Following preparation, an inguinal incision was made, and a Tipe 3 Amyand’s hernia was diagnosed. The postoperative outcome was favorable. Conclusions: Amyand's hernia should be considered as a cause of acute scrotum in pediatrics. Depending on its intraoperative classification, treatment can be performed via the inguinal approach.
