Dilated cardiomyopathy at the end of pregnancy

Abstract

Background: Peripartum cardiomyopathy is a rare disorder of late pregnancy and puerperium, with potentially lethal consequences. The clinical picture is that of heart failure, which can be difficult to diagnose because the symptoms are often similar to those of a normal pregnancy. The etiology is unknown, although it may be related to alterations in prolactin processing, angiogenic imbalance, and inflammatory or immunological processes. Objective: To show how the diagnosis was made and the management followed in a pregnant woman treated at Cheiko General Hospital, in the Republic of Djibouti, with a diagnosis of dilated cardiomyopathy. Clinical case: A 38-year-old pregnant woman, with a gestational age of 36.6 weeks, who goes to the emergency department due to high blood pressure levels. A few hours later she begins to show symptoms of heart failure and during investigations she is diagnosed with dilated cardiomyopathy. The cardiomegaly returned considerably, as did the pulmonary congestion, so on the fifth day the patient returned to the obstetrics service. On the 11th day the patient was discharged from the hospital with follow-up by cardiology and with her baby in good condition. Conclusions: If a timely diagnosis of this serious condition in pregnant women is achieved, and early and intensive treatment is imposed, it is possible to improve the maternal-fetal perinatal results.
